The following questions are designed to help us determine a suitable match between a Darkwind puppy and you. If you have additional information that you would like to tell us, please use additional paper--we are very interested in anything you'd like us to know. Feel free to use separate pieces of paper for your answers--you needn't try to squeeze the answer between the lines. We want as much information from you as you care to give. Please return this questionnaire to us at the address below, along with the deposit payment and completed deposit form, and we will review and consider your deposit request.
